Clinical Review of 402 Cases of Stomach Cancer

Abstract
From Jan. 1960 to Aug. 1968, 402 cases of gastric cancer underwent some operation at the Dept. of Surgery, Kyung Pook National University Hospital, and a clinical review was made.
The results are as follows;
1) The peak age incidence was 5th decade in both sexes, 50 years and 2 months in average, ranging from 21 to 75 years old of age.
2) Male predominated over females by the ratio of 2.4 : 1.
3) The most frequent complaints were epigastric pain and discomfort 93.4%, epigastric fullness 57.9%, indigestion 56.2%, palpable mass 62.9%, loss of weight 56.9% and vomiting 46.7%.
4) Most of the patients (50.3%) had had symptoms less than 6 months.
5) Past histories were analyzed as follows; gastrointestinal disorder 33.8%, peptic ulcer 4.9%, and no particular 4.5%. In 17.4% no records were made regarding the past history.
6) Hemoglobin level was checked in 344 cases and 71.9% of them were anemic(below 11.gm.%)
7) Gastric juice analysis was done on 140 cases, 72.9% of them fell in the range of either achlorhydria or hypochlorhydria.
8) Upper gastrointestinal series was done on 252 cases and positive diagnosis (filling defect, etc.) was made on 53.9%, suspected diagnosis in 41.2% and false negative in 5 4%, respectively.
9) 86.4% of preoperative diagnosis proved correct and remaining 13.6% were wrong, which were either misdiagnosed or the coexistence of stomach cancer was overlooked.
10) The operability was 88.0%, the resectability was 55.0% and the radical surgery was done in 50.8% of total cases.
11) The locations of the tumor in the order of frequency were as follows; 31.7% at the antrum, 28.5% at the pylorus, 24.3% at the lesser curvature and 6.2% at the greater curvature.
12) The most frequent site of metastases was the regional nodes (81.4%) and the rest were the omentum (39.1%), mesocolon (22.6%), pancreas (13.7%), mesentery (10.7%) and biliary tract (4.9%).
13) Most of the patients (86.3%) stayed in the hospital less than 3 weeks after surgery.
14) Postoperative complication developed in 13.4% and most frequent ones were wound infection (6.2%), dumping syndrome (1.9%), wound disruption (1.9%) and stomal bleeding (1.2%), Hospital death was 5.2%.
15) Histopathological classification was as follows;
adenocarcinoma 69.8%, mucinous type 10.3% anaplastic type 8.2%, scirrhous type 7.0%, papillary type 2.5% and medullary type 2.1% in the order of frequency.
16) Long term follow up of 194 cases of the total are being carried out and the analysis will be reported in near future.
